What You’ll Do
- Deliver skilled nursing care to patients in their homes throughout assigned communities
- Develop and manage plans of care in collaboration with physicians and interdisciplinary partners
- Provide patient and family education that promotes independence and recovery
- Complete required documentation (OASIS, visit notes, orders) electronically using a provided laptop
- Contribute to high-quality, patient-centered outcomes while maintaining regulatory standards

Qualifications
- Graduate of an accredited school of professional nursing
- Active RN license (Pennsylvania or compact state eligibility)
- CPR certification (AHA) within 30 days of hire
- Valid driver’s license, auto insurance, and required clearances
- Final title and compensation are based on experience, education, and career ladder placement.

- Current licensure as a Registered Professional Nurse either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located OR TPP
- CPR is required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
