UPMC East is currently seeking an organized and detail-minded individual to fill a part-time Medical History Specialist role.  This position will be scheduled 24 hours each week (three 8-hour shifts, 10 am to 6 pm) and will have rotating Saturday responsibility.

Purpose:
Under the direct supervision of the Emergency Department Director and Clinical Pharmacy staff, the Medical History Specialist interviews patients and reviews medication-related documentation for the purpose of compiling a medication history for patients being admitted to the hospital. Compiled history is then entered into the patient’s electronic health record for the purpose of facilitating the medication reconciliation process by the admitting provider. May perform a variety of functions which impact the medication reconciliation process and patient care. Uses the Clinical Pharmacist as a resource for any issues/concerns arising with the medication reconciliation process.

Responsibilities:

  • Constructively receives feedback and direction. Identifies and communicates learning needs to the unit director. Takes action to improve knowledge, skills, and performance. Requests assistance in planning and prioritizing activities as needed. Participates in self-review as requested by the unit director.
  • Answers inquiries by providing accurate information or forwarding the callers to the appropriate resource. Identifies urgent messages, directs information to the appropriate person and maintains confidentiality.
  • Communicates any delays/difficulties in the reconciliation process with the appropriate disciplines. Follows up on these delays/difficulties in order to troubleshoot issues in the future.
  • Directs any unresolved issues to the admitting provider.
  • Effectively communicates with other healthcare providers to prioritize patient care and identify patient specific needs.
  • Updates the medication list in a timely fashion so it can be used by the admitting provider as the basis for the patient's admission medication orders.
  • Gathers information to complete the best possible medication list from various resources including but not limited to: Patient interview, physician offices, community pharmacies, historical encounters, extended care facilities, or other acute care facilities. This is done to ensure complete and accurate entry into the hospital's electronic health record. This information is utilized by the admitting provider as the foundation for accurate admission orders.
  • High School Diploma or equivalent. 
  • Two or more years of experience in related role preferred.

Licensure, Certifications, and Clearances:

  • Certified Pharmacy Technician (CPhT) preferred
  • PA State Board of Pharmacy Registration required for all PA locations
  • Act 31 Child Abuse Reporting with renewal
  • Act 33 with renewal
  • Act 34 with renewal
  • Act 73 FBI Clearance with renewal
